ALL 1st Time Registrants
Starting fall 2023
If you are registering a new player to BASC or a player that has not played for BASC since before fall 2020, a copy of their birth certificate will be required to be uploaded during registration.
If you do not have a copy of your players birth certificate, email birthcertificate@basoccer.club for additional help.
If you are having trouble locating the original birth certificate, please click here for information regarding replacement of a lost state of Oklahoma birth certificate.
As a parent or guardian of a foreign born player, US Soccer and FIFA require you to complete additional forms and submit supporting documentation for your child to play soccer in the USA. This is not a rule set by Broken Arrow Soccer Club. We are here to help you understand the process the best we can, but we are not able to approve your child. This process needs to be started within 7 days of your player’s registration. This requirement applies to your player regardless of their citizenship status. If your player was adopted from a foreign country, born on a foreign US military base, or born in a foreign country (even if you are a USA citizen), US Soccer and FIFA require you to complete this process.
Registration Forms and Requirements
Follow this link to review and choose the process should be used depending on your player’s age and when they entered the USA and their current status.
All required and completed documentation should be emailed to birthcertificate@basoccer.club. We will then fill out the Club portion of the application form and send it to US Soccer and copy you on all correspondence.
Please note that BASC needs approval from US Soccer before the soccer season starts in order for your child to be eligible to play in the upcoming season. (The approval process usually takes less than a week.) If issues arise getting and/or filling out the the appropriate documents, please let us know.
